The trap door was replaced with a newly designed enclosure on august 28th, 2024, and the trap was recommissioned on september 19th, 2024. Updates made to this report: -updated total costs -updated this narrative above incident summary: on tuesday, july 30, 2024, a crew arrived onsite to remove a cleaning pig from the scraper trap door at the texas city scraper trap site. (in accordance with a 3rd party lease agreement). The crew completed loto/drain-up, opened the trap door, installed a new o-ring per procedures, and closed the trap door. The line was restarted on july 31st per schedule and a third-party operator identified that the trap door, which is located within a third-party facility, was leaking. The line was immediately shutdown after identification. The release was contained all onsite and cleaned up. Root cause: the scraper trap door pinched/stressed the o-ring at the 12 o'clock position causing failure of the seal and release of crude oil. Contributing factors: o-ring position on threads increases likelihood of damage during door closure. Preventing re-occurrence: the scraper trap door was replaced on august 28th, 2024, with a new trap door that will reduce the likelihood of a pinched o-ring during closure.
