Narrative
Phillips 66 bartlesville control center notified local operations of a leak detection alarm on ph50 pipeline. Local operations investigated the notification and discovered the leaking body bleed on the block valve. This tubing is part of the body relief system of the wkm pow r seal valve, which discharges into the upstream side of the pipe. The laboratory analysis confirmed the tubing failed due to high cycle fatigue from low stress cyclic stress. Corrective actions: 1. Replace the body heavy (mass) carbon steel fittings with lightweight stainless-steel instrumentation fittings. 2. Install tubing support to minimize vibration. Supplemental: corrected local time of confirmed discovery. Corrected type of material involved. Corrected downstream valve type used to initially isolate release source.
