Narrative
Ngl tech arrived at the sandhills pinto creek trap receiver on 7/18/24 @ 8:42 am for scheduled routine maintenance. The ngl tech immediately noticed the closure door on the pig receiver trap had an ice ball on it. The tech backed off and made notifications to field supervision then isolated the receiver trap to stop the leak @ 8:51 am. The leak amount was calculated by engineering and determined to be reportable to the trrc. The leak was reported to the trrc 24 hour emergency line @ 9:20 am. Root cause: investigation and lab analysis indicated that probable root cause was damage to o-ring when door was last closed. Misalignment of door caused the o-ring to be slightly pinched upon closure. Corrective action: weldfit service department was called to examine the closure door. The weldfit service tech adjusted the door for realignment on 7/31/24. Update.
