Narrative
A truck driver notified company personnel of a tubing failure inside sunoco's laredo terminal. The failure originated from a 1/2 inch tubing fitting connecting a pipeline pressure transmitter adjacent to the launcher trap on the dos laredo segment of the 50p laredo pipeline system. Company terminal personnel were alerted and immediately responded to trap site. The leak was isolated using a valve below the fitting, and the tubing and fitting was removed and preserved for analysis. The transmitter was re-mounted directly to the pipeline fitting above the isolation valve. The associated tubing and connection will be examined to determine the cause of failure. Update - the leak was the result of mechanical damage or crimping of the tube, during installation, that allowed movement of the ferrule. The exact date of the instrument tubing installation is unknown and is believed to be original to the segment's construction in 2004.
