Narrative
On 7/13/24 at approximately 04:00, a crude oil release occurred within the texoma manifold. Operations personnel were immediately dispatched to the area of the release to assess the situation and begin cleanup. It was determined that tank 1541 tank gate valve was not opened in sequence, which caused an overpressure of the seaway 30-inch transfer line. The pipeline overpressure caused the shutdown valve to block in automatically and stop the flow of crude. Approximately one (1) barrel of crude oil was released. On-site contractors cleaned-up released product. Gaskets were replaced. All flanges were torqued to spec and checked for leaks. The line was static pressure tested and was monitored for leaks prior to restart. The cause of the leak was an overpressure due to the local facility control room operator's failure to follow procedure. Operator was sent for drug and alcohol testing (negative) and oq was suspended and the individual will be retrained to prevent reoccurrence.
