On june 19, 2024 at approximately 11:34 a.M., field personnel were on-site monitoring a recently installed vibration sensor when a 1" diameter schedule 80 pipe used for thermal relief off the discharge side of mainline pump unit 403 ruptured. Field personnel witnessed the pipe vibrating when the pumps were shutdown, resulting in the release and immediately notified the control room and notified a cenex pipeline qi. Since the segment was already shutdown, the controller further isolated the segment by closing the nearest upstream and downstream remotely operated valves. There was approximately 1 barrel of diesel released which was captured for disposal. Cenex pipeline notified the north dakota department of environmental quality of this event. The failed connection was sent to a third-party vendor for failure analysis. The root cause of this incident was identified as fatigue cracking of a threaded/back welded connection. Contributing factors included vibration and inadequate support. The relief line was replaced with adequately supported stainless tubing and the associated pump is being rebuilt and rebalanced. Additional corrective actions related to this incident are underway.
