On june 14th, 2024, harvest operator arrived at cocodrie station for routine station visit at approximately 7:00am. Upon arrival, harvest operator heard tk-1352 mixer making noise and discovered leak which released about 25 barrels of oil to the ground within tk-1352 containment area and no oil travelled off site.. Harvest operator called harvest control room and harvest senior foreman regarding tk-1352 mixer leak. Harvest operator informed harvest control room and harvest senior foreman that cocodrie station has been isolated from inbound crude oil volumes. Harvest senior foreman made notifications at 7:08am to harvest operations manager and harvest environmental, health, and safety (ehs) specialist. Harvest senior foreman notified louisiana state police spill center and report #24-02082 was filed. At 7:40am, harvest construction supervisor contacted third party contractors for clean up work. At 8:30am, harvest senior foreman notifies national response center (nrc) and report #1401826 was filed. At 8:44am osro arrives on site to do clean up effort. 9:00am - containment was installed under tk-1352 mixer. Oil was then pumped into the sump and pumped out to pipeline. At 9:15am, harvest senior foreman arrived at cocodrie station. Harvest operator and harvest senior foreman discussed isolation of tk-1352. Harvest senior regulatory and compliance specialist notifies ldnr about tk-1352 mixer leak. Harvest senior regulatory and compliance specialist also notifies ldnr assistant director about tk-1352 mixer leak. At 9:45am, harvest operations isolated tk-1352 and directed crude oil volumes to pipeline. At 10:00am, harvest operations continued isolation of tk-1352 and begins to use tk-1355 for incoming crude oil storage. From 10:00am - 6:05pm, clean up efforts continued, free liquids are vacuumed, and contaminated soil is collected for disposal. At 6:05pm, harvest operations pumped down tk-1352 below mixer to stop the leak. Tk-1352 mixer is removed and tagged for equipment inspection. Harvest operations has removed tk-1352 from service until tank integrity can be determined. On july 31, 2024 a supplemental report was filed to clarify the associated lab costs under 8f and to update the pressure at the mixer at the time of the accident which was 3.5 psig on part e #1. Part a 17b requires a date and time for bringing the equipment back into service. Currently there are no plans to bring the equipment back into service during 2024 due to operational conditions outside of this accident. As of december 2024, the tank remains out of service. Plans are to permanently remove the tank from service in 2025.
