Narrative
On june 12, 2024, a release of 2 barrels of gasoline occurred from a maintenance activity that was completed on a flange connection on june 10, 2024, by two employees. The release was completely contained on the property and any petroleum impacted soil was properly managed. Both employees were requalified for the covered task associated with the maintenance activity. Part f ' drug & alcohol (d&a) testing information the maintenance work was associated with the installation of a blind on a flanged connection that ended june 10, 2024, at 18:00. However, the release did not occur until june 12, 2024, when it was discovered at 17:55. Therefore, as the event occurred >32-hours after the pipeline maintenance work ended, per 49 cfr 199 no d&a testing was conducted. Part g ' incorrect operations the maintenance operation involved the installation of a blind, and the appropriate torque indicated in the torque procedure for flanged pipeline components was not correctly applied. As this task is considered an oq covered task at explorer pipeline, the two technicians associated with the project were removed for a oq performance verification re-qualification by an oq team. An additional technician, not associated with the june 10, 2024, project conducted the appropriate torque.
