Narrative
On june 3, 2024 at approximately 11:50 a.M., product ignited during a pigging operation at west texas ngl pipeline's orla injection site. A 3rd party is performing a cause failure analysis and will subject the relevant equipment to lab analysis. The results will be shared with PHMSA and oneok will supplement this report as appropriate. Supplemental 2/17/25: the third-party causal report identified that the associated valves leaked due to mechanical damage to the valve seals. The damage was a combination of wear from the valves opening and closing, and particulate materials scratching and embedding in the seals. Failure to follow the hot work standard by allowing a non-intrinsically safe device into the work area with a potentially hazardous atmosphere has also been identified as a contributing factor to the event. The release and subsequent ignition resulted in two contractor injuries. A line stop on the associated pipeline was utilized to make the required repairs.
