Narrative
On may 25th, 2024 at 10:35 cst, kmlt received a scada alarm immediately after starting up pump 3065. The controller immediately shut down the system and isolated the tank. At 10:36 cst a crew leader in the area confirmed that the sight glass on the 2" maintenance start up and shut down system, was leaking into concrete containment. The area was cleaned via a sump pump and the sight glass was removed and air gapped. The component has been replaced. Root cause investigation did not yield a determining factor for the failure at the sight glass.
