Narrative
On may 19th, 2024, field operations were refilling a new line that was partially drained to complete work activities. As gasoline was pushed through the line, valve 801-1 began to release onto the ground. As the movement and release were ongoing, operations returned to the area for a different task and detected a gasoline odor. All movements were shut down, the onsite investigation revealed that valve 801-1, which was installed 10 days prior by a contractor, had been installed and left in the open position, uncapped. An osro was notified, and the release was cleaned up promptly. Supplemental update: removed the word parish from county/parish field.
