Narrative
On monday, may 13th, 2024 at approximately 03:11 a local operator discovered a release of gasoline from a flange on the l7918 inside the cenovus refinery. The line was not running but the leak was isolated in coordination local operations, mt. Belvieu control center, and cenovus refinery operations. There were no injuries, fires, or offsite product to report with this release. The root cause was determined to be failure to follow procedures and installation of an incorrect gasket.
