Narrative
11/18/2024 at 13:30 hours, a pipeline operator was conducting a routine inspection and discovered a leak on pipeline m2115-1. The pipeline was isolated and less than 3 mmcf was released. The pipeline was excavated, and a crack was located near in the heat affected zone of a fillet weld at the 6 o'clock position. The crack was on the 30" mainline piping adjacent to a spherical tee that was installed for a pipeline hot tap. Determination was made that the repair would exceed the reporting threshold of $145,400 and notification was made to the nrc at 11:20 am on 11/19/24. The spherical tee was removed, and a barred tee was installed on sunday,11/24/24. The pipeline was placed back into service on monday, 11/25/24. A third-party failure analysis was performed on the component. The root cause was identified as an issue in the welding process. Contributing factors included insufficient pre-heat and lack of cleanliness in the fillet weld.
