Narrative
Approximately 6 barrels of crude oil were released from sump 2090 at oryx carlsbad station. The date and time of the release (part a q4) are indicative of the earliest possible date/time the release could have started based on control center information. The cause of the release is attributed to equipment failure of a check valve likely as a result of debris caught in the check valve, allowing the sump to overfill. Additionally, the sump hi-hi alarm failed to alarm due to program settings that were not re-set following testing. To prevent future incidents, the check valve was replaced and the programming on the sump hi-hi alarm was reset, tested, and verified. Supplemental narrative (1): updates applied to question 8 in part d.
