Narrative
On july 2, 2024, a fire occurred at the oasis waha compressor station, 2.26 miles north of coyanosa, texas in pecos county. At approximately 0500 scada notified operations about a unit down on fire detection. 0520, it was discovered that compressor units 3 & 4 were on fire, esdd the station and called the local fire department to the station. At 0543, it was determined the repairs and damages would exceed the $145,400.00 reportable dollar criteria and the nrc and the txrrc was notified. Local fire department extinguished the fire. The damaged compressors were bypassed, and the station was back online on july 5. Fuel gas and line oil fueled the fire. The cause of the fire is still under investigation. A supplemental report will be submitted once investigation is complete. **during the physical inspection there were also found to be broken bolts gathered for analysis. The coupling cover bolt, as well as a missing bolt from the turbine drain flange were found on the skid floor. These bolts were also sent to metallurgist also and were determined to have failed due to fatigue. The sheared bolt and staining on the drain flange suggest a possible root cause of the fire. The failure of the flange as an oil sealing mechanism would lead to oil draining directly onto the exhaust blanket leading to vaporization and ignition of the oil. This was an isolated incident. The mitigation steps to prevent this from occurring in the future are as followed: -evaluate fire eyes/gas detector placement on these packages. -change fasteners on engine swings. -coordinate an evaluation of all units to confirm no missing/broken fasteners, this is now included in our 4000-hour preventative maintenance plan.
