On 05/31/2024 at 18:12, the 12-inch station discharge header blowdown inadvertently opened resulting in gas blowing at approximately 1100 psi for 78 minutes. A local emergency responder notified the control center in houston, tx. Control center called local operations management and an fgt employee was dispatched to compressor station 27 thonotosassa and found the discharge blowdown open. The employee immediately closed the manual block valve just downstream of the automatic valve. Upon further investigation, the employee found a three-way valve on the esd line in the closed position. Seals in the operator on the discharge blowdown valve had been replaced on the previous day. There are two valves inline on the esd sensing feed line. The cause of the incident was due to a valve left or placed in wrong position, but not resulting in an overpressure. Replacing 3-way-switching valve with a manual block valve in line with a manual bleed between the two block valves to prevent potential reoccurrence.
