After reviewing the timeline of events and the factual evidence, the investigation team concluded that while there was not definitive evidence that circulation of the well would have prevented this incident, it appears that the gas release occurred as a result of well fluid imbalance. Had the well remediation process required circulation of the well fluid, conditions in the well may not have reached a state of imbalance, which ultimately led to the loss of well control. Therefore, the well remediation procedure was determined to be inadequate. The full root cause failure analysis was submitted to PHMSA on november 19, 2024.
