Narrative
Coserv gas, ltd. Employees were performing a hot tap on an in-situ steel gas line at the above-described location of incident. They were using a mueller co. Manufactured hot tapping tool as a part of this work. The ratchet handle of the tool's mounted completion machine moved unexpectedly as a result of some yet-to-be-determined failure internal to the tool. A coserv gas, ltd. Employee was struck by the ratchet handle and suffered fatal injuries. There was no resulting gas leak.
