Narrative
On july 10, 2024, a second-party contractor was dispatched to repair a leak on a service tapping tee in the alley of northlake drive in richardson, texas. At the time of the repair, both the main and tapping tee were steel. To retire the tapping tee, the cap was removed and a small quantity of gas was released. At approximately 3:30 p.M. A flash fire occurred. The source of the ignition is unknown. An employee onsite to observe the repair notified atmos energy of the flash fire. Contractor personnel extinguished the fire. The injured contractor suffered burns to his hands, was transported to the hospital, and was admitted at approximately 6:30 p.M. The hospitalization is confirmed discovery time for this event. The leak repair was completed at 1:17 a.M. On july 11, 2024. The equipment being utilized was appropriate at the time. After the event, a stand down was conducted of similar work across the division and practices, procedures, and materials were reviewed for potential improvements to avoid re-occurrence.
