On april 5, 2024, at approximately 13:07 pm, a 10-inch steel high priority gas main was damaged by a third-party contractor attempting to drive a steel sheet pile into the ground, subsequently striking the gas facility. A locate ticket from the primary contractor received a positive response from pgs on april 2nd - sunshine 811 positive response code 3m. We understand that the primary contractor who contacted the one-call center, memorialized in ticket number 088406955, was not the excavator who performed the work. Any sub-contractors performing work should have called in their own locate request, of which none were found for this excavation work. Preliminary investigation did not find where the primary contractor contacted pgs prior to excavation. Additionally, a vertical pipeline marker was present and visible to the excavator at the time of the incident at the excavation site. The damage was made safe at approximately 04:24am on 04/15/2024 by constructing a permanent offset. All permanent repairs and restoration activities have been completed. Final estimated costs have been updated in this final report.
