Acc- pl2024-0058 on april 15, 2024 at 10:35 southwest gas (swg) dispatch was notified by the phoenix fire department of a vehicle that had collided with regulator station (dr #423886) at the northwest corner of 27th avenue and northern avenue in phoenix, arizona. Construction crews responded and discovered a multiple-vehicle accident had caused one vehicle to collide with the regulator station and sheared off the above-ground 2-inch bypass pipe from the regulator station resulting in a release of gas. Gas flow was controlled utilizing three valves to isolate the damaged regulator station and gas off at 12:22. Approximately 555 individuals were evacuated from a school as a precaution by the fire department. Three people were transported to the hospital due to minor injuries sustained in the vehicle accidents. There were no outages, or fatalities due to the gas incident. The district regulator station was temporarily taken out of service for replacement / relocation at a later date. On april 15, 2024, the arizona corporation commission (acc) was telephonically notified at 11:27 of the incident involving the evacuation of the school and that estimated incident costs would exceed $139,700. On april 15, 2024, PHMSA/nrc was telephonically notified at 11:40 that incident costs would exceed $139,700 and was assigned nrc report #1396485. On april 16, 2024, a follow-up PHMSA/nrc was telephonically updated at 14:11 and was assigned nrc report #1396605.
