Narrative
At approximately 6:00 am, on march 20, 2024, questar gas company d/b/a dominion energy utah (deu) received a call from 911 reporting a residential explosion at 67 north 370 east, american fork, utah. Deu responded to the scene and observed an active fire in the duplex structure. American fork fire department shut off the flow of natural gas. It was reported that the resident of 69 north 370 east was found deceased. The structure at 67 and 69 north 370 east were destroyed. The home at 53 north 370 east sustained structural damage. Once the scene was secure, deu performed pressure tests on the main and service lines associated with 67 and 69 north 370 east. 67 and 69 north 370 east were served by a single service line and dual meter sets. Deu tested the main and service lines at operating pressure of 45 psi for 15 minutes. Each held pressure, indicating no leaks on the main and service lines. These pressure tests were witnessed by a representative of utah pipeline safety. Deu also executed its leak screen procedure, screening for leaks on the other main and service lines in the neighborhood and found no leaks. Deu's cause and origin investigation is ongoing. At the fire departments direction, deu removed the meter sets and riser for 67 and 69 north 370 east and surrendered them to the fire departments custody on the day of the incident . Testing of our deu equipment was completed on 7/29/2024. Initial nrc incident report #1394296, was made on 03/20/2024 at 7:42am mt. 48-hour update nrc #1394442, was made on 03/21/2024 at 7:30pm mt.
