Narrative
Approximately three barrels of crude oil were released from the gardendale to three rivers loop line 20inch coupon at three rivers terminal. The date and time of the release (part a q4) is indicative of the earliest possible time the release could have started based on pressure trends. The cause of the release is attributed to incorrect operation as a result of improper installation of the coupon plug. To prevent future incidents, plains suspended the oq qualification of the third-party individual that performed the installation, and new fittings and plug for the coupon were installed.
