On november 16th, 2023 at 12:50 operations personnel conducting daily facility inspections observed a small sheen and stained soil in tank 1653's valve vault with no signs of an active leak. The affected segment was isolated at the time of discovery and cleanup activities were initiated. During cleanup activities, the volume was determined to likely be more than 5 gallons and the trrc was notified at 13:36. Further investigation determined extreme temperature swings caused expansion and contraction of the bolt tension resulting in the torque values relaxing which allowed product to seep through the flanged connection. To prevent a reoccurrence the affected flange, and similarly situated flanges were torqued to the upper end of the permissible range.
