Narrative
On 11/18/2023 at 11:38 cst an enterprise pipeline technician arrived at coupland pump station on the midland to sealy 2 system and observed smoke and fire coming from unit 3100. Simultaneously, scada alarms from the facility's fire detectors occurred at the houston control center, shutting down the station. The pipeline technician used a fire extinguisher to put out the fire, which was extinguished by 11:44. The pump station was bypassed and the line remained in operation. Pump repairs were completed and the pump was returned to service on 11/29/2023 at 14:08 cst. The pump seal failure appears to have been caused by improper installation. It is believed that set screws had not been sufficiently tightened, and the pump seal shifted after approximately 520 hours in service. To prevent recurrence, the applicable maintenance procedure was updated to instruct personnel regarding proper set screw torque values to be used.
