Narrative
On 11/20/2023, a piece of tubing came loose on a check valve on a pump at boardwalk ethane pipeline company, llc's (boardwalk) port aurthur pump station. An alarm was received for p200 at 14:30. At 14:56, operations personnel arrived onsite and confirmed the release. The release was contained by valve operation. At approximately 15:48, the broken fitting was removed and the piping was capped. The suction header was pressured up and p100, which was offline at the time of the accident, was started; p200 was locked out. It was determined that the tubing broke loose due to excessive vibration. Boardwalk will install a pipe support to prevent this from occurring in the future as well as evaluate the piping configuration to determine whether modifications are needed.
