Narrative
On november 7, 2023, at approximately 10:35 am cst, operations personnel noticed the smell of crude oil at the gray oak ingleside station and discovered oil dripping from the threaded fitting of the sample probe on the east meter skid. The control center was contacted, and the line segment was shut down. Approximately 5 gallons of crude oil was released. The railroad commission of texas was notified at 11:00 am cst (report # 2973). The line segment was drained, and the sample probe was replaced. All contaminated soil was removed from the release site and properly disposed of. The investigation determined the cause of the release was due to a defective threaded flange. The flange was not fully threaded by the manufacturer and the defect was not found prior to installation on november 1, 2023.
