Narrative
The phillips 66 bartlesville control center received an emergency line call at 13:37, 11/2/23. The caller reported petroleum product escaping from an excavation. This call was made by the contractor who was operating an auger directly over the gd03 line. The line was shut down and blocked in within 6 minutes of the commencement of the emergency line call. There was no active one-call ticket for the auger work, thus p66 personnel were not on site. The auger pierced the pipe very close to the 12:00 position. The pipe has apx 84" of dirt cover at the incident location. The line was initially blocked in with two remote operated valves' positions separated by apx 14 miles. Local p66 personnel arrived and manually closed two additional valves that reduced the affected pipeline distance to apx 4 miles. This occurred in a p66 r-o-w through a baseball complex that is under construction. The line was excavated, contaminated soil removed and a repair clamp was installed. The line was returned to service on 11/4/23 at 15:35. 12/4/23 - part c, item #3a - changed from 12.75" dia to 12" nominal pipe dia, per PHMSA request. 12/4/23 - part e, items #9c & 10c - changed from "no" to "yes" at the insistence of PHMSA, since "yes" was normally what they see, though "no" accurately reflects accident conditions. 2/13/24 - part a, item #25 - updated public evac total to 661 after receiving data from the local fire department. 2/13/24 - part d, items #8c, 8d & 8e - updated accident costs 2/15/24 - part e, items 9c & 10c were changed back to the accurate response of "no" after discussion with PHMSA.
