At approximately 20:00 on october 18th, 2023 operations personnel conducting facility inspections noticed the smell of product. At 00:30 on 10/19, personnel found product releasing from relief tubing in the southwest vault. The affected segment was isolated and clean-up activities were initiated. Further investigation found the relief tubing going into the ferrule had sheered, likely due to excessive vibration and unsupported tubing. To prevent a re-occurrence, the tubing was replaced and additional supports were added.
