Narrative
While make a pipeline transfer into tank 2307 a contract software programmer inadvertently closed the motor operated valve at the inlet to the tank. The software programmer did not know the control room operator was making a transfer into tank 2307 at the time. The programmer did not check with the control room and assumed no transfers. Investigation into the causes of the incident fatigue was not determined to be an issue. The control room operator had the system lined up according to procedures. The third-party programmer was at fault for not reporting into the control room operator prior to beginning work. Procedure has been updated for anyone doing any work associated with the control room is now required to obtain a safe work permit from the control room operator and sign by the operation's supervisor prior to beginning work. As noted, drug and alcohol testing were not collected. The site is new to the requirements and did not realize the requirement. Since the incident jefferson is establishing the drug and alcohol testing instructions to supervision
