Narrative
At 07:35 on october 15th, 2023 operations personnel conducting facility inspections at el dorado east tank farm found product leaking from a flanged connection on the tank 1211 piping discharging from unit 1211a. Upon discovery, the operator switched the operation to a different pump out of the tank, isolated the leak point, and initiated clean-up activities. Tank 1211 piping had been taken out of service for maintenance the week prior. The tank and piping were placed back in service on the morning 10/14 and showed no signs of a release during the inspection on the evening of 10/14. Further investigation found the gasket located between the flanges of a check valve and a closed manual maintenance valve had failed. The project required the valve to be closed to act as a point of isolation and was opened after the line segment was refilled with product. The time between the line being refilled and the opening of the valve allowed thermal stress to damage the gasket although not to the point of immediate failure. The gasket failed after a day of typical operating conditions due to the thermal stress sustained during maintenance weaking the integrity of the gasket.
