Narrative
On 10/11/2023 at approximately 20:55 cdst operations discovered ultra-low sulfur diesel coming from a 1" high point ball valve on the p113 meter skid. In response, the active product movement was shut down and the leak point was isolated. Approximately 30 gallons of ultra-low sulfur diesel were released and have since been cleaned up. It was determined that personnel failed to close the 1" valve following maintenance on the meter skid. Oq's were suspended and retraining was held for the personnel involved.
