On october 4th, 2023 at 14:30 operations personnel at olathe station performing preventative maintenance activities discovered product leaking from the valve stem of mov2 on the paola - kansas city #5-12" pipeline. The line was shut down and kept open into the kansas city terminal to allow product to drain downstream. Clean-up activities were initiated. Further investigation found an o-ring had failed. To prevent a re-occurrence the 1950 installed valve was replaced by a valve with injectable packing. 5/2024 supplemental/final: part d
