On september 29th, 2023 at 11:00 magellan operation personnel at the east houston terminal placing tank 2501 back into service discovered product coming from an open 2" valve on the tank lateral. Personnel immediately closed the valve, installed a plug, and clean-up efforts began. Tank 2501 had been out of service for routine and other maintenance for roughly a year and a half. To prevent a reoccurrence, procedure 9.01-adm-045 - perform valve lineups for product movements will be reviewed during the next safety meeting with east houston terminal operations and maintenance teams, with emphasis on valve alignment. Additionally, streamers and tags will be placed on all valves and fittings being operated during maintenance.
