Narrative
The harvest operator arrived at three rivers station and upon arrival discovered oil near pump. It appeared to be from a spray mist blown across the pump and motor. The harvest operator notified the control room to shut down the pump and close the upstream and downstream valves. While depressurizing the pipe, a small crack started leaking from the weld-o-let. Corrective actions to minimize recoccurance- harvest will remove 1' flanged ball valves on mainline pumps at three rivers station and replace with blind flanges. This will remove the excess weight on the 1' piping. Harvest will complete an evaluation of 1' piping with similar design to p-5100 mainline pump at three rivers station using suitable ndt methods.
