Narrative
On september 25, 2023, a technician was conducting a facility check at the wichita falls terminal when he noticed a product leak at the pig receiver door. The technician promptly reported this to their supervisor, the hep control center, and initiated contractor assistance for cleanup. The valves isolating the pig receiver from the mainline were closed when the leak was discovered, and the mainline was not in operation (specifically, it was not scheduled to be delivering into the terminal at that time). As of this initial report, the exact volume of the recovered product is pending determination. As a result of the leak, agency notifications were made to the national response center (nrc), the texas railroad commission (rrc) and the texas commission on environmental quality (tceq). Upon investigation, the root cause of the release appears to be a damaged o-ring. Upon removal, it was observed that the o-ring had multiple nicks, which likely led to the leak. At the time of this report, both the pig receiver door o-ring (the source of the leak) and a secondary o-ring on the vent cap (which was not found to be leaking) have been replaced. 12.12.2023 update: the final recovered volume for this release was determined to be 2.1 bbls. Additionally, it has been confirmed that a failed o-ring was the cause of the release.
