On september 25, 2023, at 10:24am, centurion pipeline, l.P. (cpl) employee was notified by a third party excavator that a bulldozer had struck one of cpl's "slaughter to drumright" right-of-way (row). The employee's supervisor (qualified individual) and control center was notified. The pipeline system was previously shutdown due to reduced tank levels at cpl's cushing station at 10:17am. Cpl personnel arrived on-site at 10:44am and confirmed discovery on the "cushing west to el reno #1" at at 11:10am. Leak was located at approximately mp 61 in oklahoma city, oklahoma in oklahoma county. The company's osro was notified at 11:00am of the leak and arrived on-site at 11:50am. The national response center (nrc) was notified at 12:18am with incident report# 1380056. One thousand, six hundred and eighty-four (1,684) barrels was released and recovered. No injuries, fatalities or fires were associated with this incident. Remediation has been completed on-site. Repairs were completed and segment was returned to service. It was determined that the third party excavator had made the properly one-call ticket and the locate procedures were properly followed. In discussions with cpl's line locator and the excavator, it was determined that the third party excavator's original plans of excavation was too close to cpl's cushing west to el reno #1 to maintain clearance. It was determined that the excavator would revise the excavation plans to include additional cover and cross above cpl's pipeline instead of underneath. The third party excavator was informed that cpl's employee would need to be on-site for any excavation. The excavator did not make notification to cpl on the day of the incident and cpl was not on-site. On the day of the incident, it was determined that excavator's point of contact had not followed the modified plans nor was aware that there was an excavation crew on site. Supplemental report filed on november 21, 2023: updated part a5: location of accident's coordinates. Updated part a17 and included shutdown and restart times. Permanent repairs have been scheduled for late november 2023 and no further actions will be required.
