Friday, september 15 at 2:00am ' mike smith received a call from coffeyville resources (cvr) plant control room with a report that the capturepoint launcher barrel was leaking CO2 based on observed cloud and CO2 blowing from the end of the barrel. Cvr had peco (contractor) completing work in a fabrication area approximately 50 yards south of capturepoint fenced area where barrel is located. 2:15am - mike smith arrived at the plant and saw the leak on launcher barrel door o-ring seal appeared to have blown out. 2:20am - 2:30am ' mike smith assessed what was happening. No one was in or around the immediate area. Took pictures and video. The wind was blowing ese 3mph, and it was 62 degrees. 2:40am-3:00am ' mike smith removed lock from kicker valve that was found in closed position. Removed lock from isolation valve that was found open. I closed the isolation valve and opened 3 - vent valves to relieve pressure from launcher barrel. 3:11am - mike smith sent 5 photos to mike godfrey, pipeline supervisor, via text message. 4:43am ' mike smith sent email to christina harvick, director of pipelines and facilities, and to mike godfrey, pipeline supervisor, with the details. 4:45am - 5am - the launcher barrel door was still leaking but wasn't blowing as hard as when initially discovered. The barrel continued to bleed off until about 6:00am when the pressure gauge read (0) zero. 6:52am - mike smith called mike godfrey, pipeline supervisor, and reported to him what was found and how valves were left. Mike godfrey informed mike smith that he would head that direction and repair. Mike smith stayed on site until the other plant operator arrived for his shift and left the plant at approximately 7am. The barrel yale head enclosure viton o-ring was found damaged due to misalignment.
