Narrative
On september 1st, 2023, at 09:29 a pilot conducting routine air patrol observed dead vegetation in the right-of-way near milepost 181.73 of the #6-12" bettendorf jct - amboy pipeline. An incident investigation was immediately initiated. Operation control confirmed the pipeline was already shutdown and closed the upstream and downstream rovs, isolating the affected segment. Local operation personnel were dispatched to the site who arrived at 10:00 and confirmed signs of distressed vegetation potentially indicative of a release. The nearest up and downstream manual valves were closed 11:45 and 11:30, respectively. Excavation confirmed evidence of a low-rate release that was repaired with a type b sleeve at approximately 18:30 on september 1st, 2023. The affected pipe joint was cut out and replaced on september 8th. The anomaly was sent for metallurgical analysis. The approved restart plan was executed successfully on the 13th and the line was approved to return to normal operations. The line resumed normal operation at 08:00 on september 14th, 2023. The metallurgical report states "the leak was caused by a through-wall flaw located at the fusion line of the erw seam. The leak feature was caused by a localized area of the erw seam that did not have sufficient heat and/or pressure to extrude the oxides that formed along the coil edges prior to seam formation, resulting in an incomplete fusion or black spot defect. Both flaws are pipe mill defects." additionally, per the report "it is most likely that the oxides broke down over time, creating the leak path". Supplemental/final (9/2024): parts a, g and h
