On 8/29/2023 at approximately 12:05 et, the control center received a call from a property owner concerning a gasoline odor and dead vegetation near the inland toledo to fostoria tr10 line. Internal notifications were made, and the pipeline was shutdown at 12:24. Control center personnel notified local operations and personnel arrived on site at approximately 12:40. After investigation and excavation the release was confirmed to be from the tr10 pipeline at approximately 15:15. Details were compiled and nrc, PHMSA and ohio epa notifications were made. Approximately 4 barrels of gasoline were released into the soil, which migrated underneath the roadway into a drainage ditch that flows to ayers creek. Field personnel and osros worked to recover free product from soil and water and remove contaminated soil. On 8/30/2023, a plidco mechanical clamp was installed as a temporary repair until the permanent repair can be made. The line was restarted under a temporary pressure reduction until permanent repair was made. On 9/12/2023, the failed section of pipe was cut out, replaced and the temporary operating pressure reduction was removed. The failed section was preserved and shipped to a 3rd party laboratory for analysis to determine failure mechanism. Supplemental report will be filed when analysis has been completed. Supplemental report: the cause of the accident was determined to be external corrosion. The results of the analysis indicate that the leak occurred at an area of external corrosion that contained axially oriented cracking. The cracking mechanism may be associated with corrosion fatigue or near neutral ph scc, or a combination thereof. To prevent reoccurrence line will be assessed with ili technology capable of detecting like and similar features to that of the failure and appropriate remedial steps will be taken based on the results of the run.
