Narrative
On 8/24/2023 at 7:45 am a line flyer reported dead vegetation along the row near am 225 on the west leg system. An enterprise field technician arrived onsite at 10:45 am and confirmed lel readings directly over lid 410 with no visible signs of product. The pipeline segment was shut down and isolated, and a crew was dispatched to excavate the pipeline. The original pipe repair was cut out and sent to a laboratory for a failure analysis study. Based on the area of dead vegetation, an estimated 75 gallons of propane had leaked from the pipeline. The pipeline was repaired and returned to service on 8/28/2023. The results of the failure analysis indicated that the leak in the longitudinal sleeve weld was caused by a fatigue crack that initiated at the weld root. This is an nrc reportable event due to the cost of repair exceeding $50,000.

The Results Of The Analysis Indicate That The Leak In The Longitudinal Sleeve Weld Was Caused By A Fatigue Crack That Initiated At The Weld Root. A Contributing Factor In The Failure Was A Wide Root Gap Of The Sleeve Edges At The Location Of The Leak. It Appears That A Cylindrical Metal Object, Possibly A Welding Electrode, Was Placed Into The Welding Groove During The Installation Of The Sleeve To Facilitate Weld Completion.
