Narrative
Background: the working foreman was at tank #909 dike area to inspect a prv. He observed discoloration of stones and wetness around the tank chime. He opened the double bottom telltale valve and found product present. He notified the supervisor and absorbent booms were deployed around the tank chime and it was confirmed that the tank dike drain valve was closed. Approximately 5 gallons of trans mix (gasoline & diesel) was released to soil inside the tank dike. Immediate corrective actions: tank #909 was isolated, and two layers of absorbent boom were placed around the tank. A project was executed to de-inventory, clean and gas free tank #909 to allow for an internal API 653 inspection. Preventing re-occurrence: an API 653 internal inspection was completed, and all necessary repairs were made to the tank bottom per API 650 standards.
