Narrative
On august 15, 2023 at 09:26, the controller received a call from a contractor stating the crew working in the area may have hit the pipeline. Immediately, hcc dispatched field personnel to investigate. At 09:32, the controller received a leak detection alarm and initiated shut down of the pipeline. Additional operations personnel were dispatched to close manual valves closer to the leak site. At 10:05, field personnel arrived near the site and observed bubbling in the water. At 11:15, enterprise determined that, due to the anticipated repair cost exceeding $50k, the nrc was notified. Review of scada data indicates the initial pressure loss occurred at 09:15. The damaged pipeline was cut-out, replaced, pressure tested and returned to service on september 19, 2023 at 07:49.
