Narrative
On 8/15/2023 at approximately 09:15, row maintenance crew identified a suspected release near the hebert to center pipeline. Later that day once the line was exposed, operations determined the leak was coming from the 21023 hebert to center 8-inch line. Approximately 10 barrels of diesel fuel released into the soil. 2 of the 10 barrels migrated into an unnamed tributary of chiamon bayou. Contractor mobilized for cleanup. All product removed from tributary and impacted soil. Visual and dye penetrant inspections showed a pinhole leak at the 6:00 oclock position on a historic repair sleeve fillet weld. The failed pipe section was removed and replaced with 122 ft. Of new pipe. On 8/18/2023 at approximately 01:00, the line was restarted. Company personnel inspected the failed section of pipe and determined the cause of the pinhole leak to be due to incomplete penetration of fillet weld on historic repair sleeve installation. The repair sleeve was installed prior to modern standard practices.
