Narrative
At approximately 08:00 on august 7, 2023, the operations group investigated an alert from the pump station's continuous monitoring system, which detects most volatile organic carbons (vocs). In reviewing the station's cameras, we found what appeared to be staining on unit 2 pump. At 08:22, the pump station was shut down and fhr operators were dispatched to the site. Fhr field personnel arrived onsite at 10:00 and discovered product on unit 2 pump base. Upon inspection of the unit 2 pump, it was determined the npt fitting was loose, causing product to leak. Local personnel were on site to clean up the released product and the npt fitting was tightened. The station was placed back into service at 17:22 and unit 2 was started back up on august 18. In the post-accident investigation, it was determined the leak originated from the cooling line's npt-threaded connection to the pump casing. The fitting was found to be only hand-tight and lacking thread sealant. In early 2022, fhr sent the unit 2 pump to a third-party vendor for repair. The vendor inspected and air pressure tested at their facility prior to shipping back to the fhr pump station. Fhr received the pump back from the vendor fully assembled with the seal cooling line installed. The assembled pump was reinstalled by fhr personnel on october 19, 2022.
