Narrative
During operator rounds, a company employee noticed product inside secondary containment. Upon investigation, it was found that product had come from the roof drain of a nearby tank. The employee initiated supervisory and regulatory notifications, as well as emergency response activities. After investigation, the release was due to a damaged roof seal from a 3rd party pipeline pigging operation that was conducted in conjunction with the decommissioning of the attached pipeline. The 3rd party pigged the line with nitrogen and inadvertently allowed nitrogen to blow by the pig and reach the tank. In turn, the rapid expansion of nitrogen in the tank caused damage to the roof seal allowing product on top of the external floating roof. All product and affected soil were recovered via vacuum truck and roll off box.
