Narrative
Crescent conducted a hydrotest on the delta north 2 pipeline prior to the incident. During the hydrotest it was determined that valves on the ssti assembly connecting the delta duck facility to crescent's dn2 pipeline did not hold pressure. A successful hydrotest was conducted after removing a spool piece from the ssti assembly and installing a blind flange. The assembly was reassembled and product was returned to the line and a 2 hour stand up test was conducted. The line was then returned to service and during site checks by the contract crew, a small sheen was observed on the water above the assembly and pipeline. Divers went down to conduct a swab test and found a couple fittings and flanges with small leaks. New valves were ordered and during the construction activities to replace the valves on the assembly, crude oil was released. All assembly valves were replaced and the pipeline has been returned to service.
