Narrative
On 7/13/23 at approximately 15:00, a mechanical tech identified product release at the big speed junction station on the 24" houston terminal pipeline. The line was already down per schedule. Mechanical tech isolated the valves on both sides of the line segment where the release occurred. Contractors and company support personnel were dispatched to excavate and locate the source of the release. Release was later identified on a station dead leg section of piping, approximately 6 ft. Underground, between big speed and little speed junctions. A clamp was installed on the line to isolate the release. Approximately, 20 barrels of crude oil was released. Product was cleaned up and contaminated soil removed. The cause of the accident is under investigation. Supplemental report: the failed section of pipe was cut out and replaced. 3rd party physical analysis determined cause to be internal corrosion due to microbial induced corrosion. Line segment has been treated with inhibitor and biocide for approximately one year. Internal treatment program will be adjusted as needed based on findings of the analysis of this failure.
