Narrative
On 07/05/23, a kinder morgan employee noticed a leak inside the wink tank farm coming off the km2 16 inch pipeline station piping and installed temporary repair to stop the leak . The leak was coming from a 8 feet long 10 inch diameter pipe connection at the bottom of the 16 inch pipeline that was blind flanged for a future connection to a tank. An engineering clamp was installed on pipeline on 07/05 and the pipeline was restarted. On 07/06 the km2 pipeline was shut down and isolated, and associated valves and equipment were locked out and tagged out for the drain down operations. On 07/09, the dead leg was cutout and replaced with a hydro tested 16 inch piece of pipe. The pipeline was restarted on 07/10. The failed cutout section of pipe has been sent off to the metallurgical lab. 09/11/2023 update : the metallurgical analysis has been completed and based on the failure analysis report, part g question responses have been updated. The cause of the pinhole leak was determined to be microbiologically influenced corrosion (mic).
