6/27/23 @ 4:22 cst p66 houston control center receives gas detection alarm from la pryor pump station and field tech notified. 5:30 am field tech arrives on site at la pryor and confirms leak inside station and was shut down immediately. On 6/29/22 corrective measures by further isolation of the pipeline inside the station completed. Stopples were installed on both sides of the leaking check valve inside the station on the 20-inch line utilizing existing tees. The check valve replaced, and the pipeline purged and returned to service. Operations determined estimated cost of repairs to exceed $50,000 at approximately 9:30 am cst on 6/27/23 and initial nrc report number 1371374 made at 9:33 am cst. Supplemental / final report 9-25-23 submitted following completion of railroad commission and internal incident investigations on 9-11-23. Valve was inspected on 7/12/23 by third party/sunbelt valve services inc. To determine cause of leak. Internal o-ring failure was determined to be root cause. Check valve was installed when la pryor pump station was built 2017-2018. The o-ring appeared to have been crimped during initial installation of the check valve gear box assembly. Corrective action / mitigative measures: pms created for 20-inch tom wheatley check valves to be performed every 6 months. (la pryor pump station) job plan created to preform semi-annual preventive maintenance checks. Maximo work order 23-1822157 generated and completed 7/13/23 for la pryor pump station 20-inch tom wheatley check valve. Same pms and work orders created and completed for: brackettville, zella, pettus and willow pump stations.
